Cooperation declaration

We declare that we have read and understood the General Terms and Conditions of AMPQ PRO sp. z o.o. sp. k. Polish law applies. The competent court in Bielsko-Biala has jurisdiction for disputes under the contractual relationship. We confirm that the information supplied in this registration is accurate and that the signatory is authorised to represent the company.

Data supplied in this form is processed for company registration, cooperation, order and contract fulfilment, contact, invoicing and accounting, handling complaints, and establishing, pursuing or defending claims. Additional direct marketing is subject to the separate optional choice below. Where applicable, data is also processed to prevent money laundering, terrorist financing and crime. You may request access, rectification, deletion, restriction, portability or objection, and withdraw consent where consent applies; you may complain to the supervisory authority. Contact the controller at the email or address above. Supplying registration data is voluntary but necessary to establish and fulfil the cooperation.

Electronic invoice declaration

We accept invoices, duplicates and corrections issued by AMPQ PRO sp. z o.o. sp. k. in electronic form to the addresses indicated in this registration. We will notify AMPQ in writing of address changes. We accept paper documents where technical or formal obstacles prevent electronic delivery. Acceptance may be withdrawn by notification to AMPQ, with effect from the day following receipt of the withdrawal.
